Registrants who cancel must request a refund of fees paid in writing. Cancellation requests received at Convention Data Services by April 8, 2015, midnight EST will be processed, less a $75 administrative fee. No refunds will be issued until 7 – 15 days post-event. No refunds will be made after April 15, 2015. Purchased meal tickets are non-refundable. Refunds will not be granted for no-shows after the conference. No refunds will be issued onsite.

Payment Details
Fully payment must be received with your registration form. Make checks payable to the American Heart Association or charge your registration to American Express, Discover, Visa or MasterCard. Registrations without complete payment will no be processed. Wire transfers and purchase orders are not accepted.

Please Note: The American Heart Association shall not be liable for cancellation of the Quality of Care and Outcomes Research 2015 Scientific Sessions caused by labor strikes, civil disorders, fires, weather conditions, or other acts of God or for any damages or losses resulting from such cancellation.

Before a registration is confirmed, member and nonmember Student/Trainee or Technicians must provide proof of status with a current license or certificate, student ID or a letter written on official letterhead and signed by department head, mentor or supervisor.  The full nonmember fee will be charged if verification is not provided by April 15, 2015.
