Academic Record & Training/Career Goals (Applicant/PI)

Applicants must create certain documents to support their application.  These documents are numbered 1 through 3 below.  After creating the word-processed document, convert it to portable document format (pdf) and upload to the Grants@Heart system.

  • Predoctoral Fellowship and Medical Student Research Program applicants must complete documents #1 and #3.
     
  • Postdoctoral Fellowship and Fellow-to-Faculty Transition Award must complete documents #2 and #3.
     
  • Mentored Clinical & Population Research Award applicants complete document #3 (Training/Career Goals).  For the Academic Record, a form is provided for your convenience.  Download the form.
     
  • Undergraduate Student Research Program applicants complete the Academic Record form provided. Download the form.
     
Pay attention to page limits. If you upload a document that exceeds the page limit or format, an error message will display on the screen.

Create a document using the following heading: ACADEMIC RECORD OF PREDOCTORAL APPLICANT.

Use the format illustrated below to list all undergraduate and graduate courses with grades. Explain the grading system and show level required for passing. State performance on Graduate Record Examination, if available.

Note: If you received your education outside of the United States, provide this information as completely as possible. Translate the course names into English, if needed, and explain the grading or evaluation system.

Address each item below.

Grade Point Average for Undergraduate Courses:

Give MCAT scores if appropriate.


Document #3 (one-page limit)

Create a document using the following heading: TRAINING/CAREER GOALS.

Describe your overall career goals and plans.  Explain how the training proposed here will enable you to reach these goals.  Identify the skills, theories, conceptual approaches, etc., to be learned or enhanced during this award.
